Social determinants of health are conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks (CDC). Most commonly these include: Housing, Employment, Income/Wealth, Health and Social Services, Education, Physical Environment, Social Environment, Public Safety, and Transportation.
As an organization focused on prevention as well as overall health and wellbeing of children, families, and communities, the Early Childhood Partnership of Adams County uses the Social Determinants of Health framework to inform in both systems/policy and programmatic design and implementation. To do so takes strong cross domain partnership and shared accountability for larger community-based outcomes.
